What Is NAD+ and Why Does It Matter?
Nicotinamide adenine dinucleotide (NAD+) is a coenzyme found in every living cell. It plays a central role in over 500 enzymatic reactions in the body, including energy metabolism, DNA repair, and cellular signaling.
NAD+ is essential for mitochondrial function — the process by which your cells convert nutrients into usable energy (ATP). It also activates sirtuins, a family of proteins that regulate aging, inflammation, and stress resistance.
The problem: NAD+ levels decline significantly with age. By your 40s, your body may have half the NAD+ it had in your 20s. This decline is associated with fatigue, cognitive fog, slower recovery, metabolic dysfunction, and accelerated aging.
Supplementing NAD+ through subcutaneous injection delivers the molecule directly into your system — bypassing the digestive losses that reduce oral supplement absorption by up to 90%.
Why Patients Choose NAD+ Therapy
Cellular Energy & Metabolism
NAD+ is required for mitochondrial ATP production — the energy currency of every cell. Patients frequently report improved sustained energy, reduced afternoon crashes, and better exercise tolerance within the first two weeks.
DNA Repair & Anti-Aging
NAD+ activates PARP enzymes and sirtuins (SIRT1-7), which repair damaged DNA and regulate the biological hallmarks of aging. This may slow cellular senescence and support tissue regeneration throughout the body.
Cognitive Clarity & Focus
The brain consumes roughly 20% of the body’s energy. NAD+ supports neuronal mitochondrial health through SIRT3 activation, which may improve mental clarity, focus, and protection against age-related cognitive decline.
Athletic Performance & Recovery
By enhancing mitochondrial efficiency and supporting lactate metabolism, NAD+ may improve endurance capacity, reduce recovery time between workouts, and support lean muscle preservation during aging.
Cardiovascular Support
NAD+ supports nitric oxide production and endothelial function — both critical for healthy blood flow, blood pressure regulation, and long-term cardiovascular health.
Immune System Optimization
NAD+ is critical for immune cell metabolism, including T-cell and B-cell function. Adequate NAD+ levels support a balanced immune response and may reduce immune senescence associated with aging.
NAD+ Injection vs. IV Infusion vs. Oral Supplements
Not all NAD+ delivery methods are equal. Here’s how subcutaneous injection compares to the alternatives.
| Self-Injectable NAD+ | IV NAD+ Infusion | Oral NAD+ / NMN | |
|---|---|---|---|
| Bioavailability | High (near 100%) | Highest (100%) | Low (5–15%) |
| Cost per session | ~$19 | $500–$1,500 | $1–$3 |
| Time per session | <2 minutes | 2–4 hours | <1 minute |
| Location | At home | Clinic only | Anywhere |
| Dosing consistency | 2x/week steady state | Peaks then drops | Minimal absorption |
| Physician oversight | Yes | Yes | No |
| Side effects | Minimal | Flushing, nausea, chest tightness | GI upset possible |
How to Inject NAD+ Subcutaneously
Subcutaneous injection is the same technique used for insulin and many other self-administered medications. It’s simple, quick, and virtually painless with the ultra-fine 31-gauge needle included in your kit.
- Wash your hands thoroughly with soap and water. Gather your vial, syringe, 18-gauge drawing needle, 31-gauge injection needle, and alcohol prep pad.
- Prepare the vial. Wipe the rubber stopper with an alcohol pad. Attach the 18-gauge needle to the syringe. Draw air equal to your dose volume (0.1 mL for 100mg), inject the air into the vial, invert, and draw your dose.
- Switch needles. Remove the 18-gauge drawing needle and replace it with the 31-gauge injection needle. This ensures a sharp, comfortable injection.
- Prep the injection site. Clean a spot on your lower abdomen (at least 2 inches from the navel) or upper outer thigh with the alcohol pad. Allow the area to air dry for 30 seconds.
- Pinch and inject. Pinch a 1–2 inch fold of skin. Insert the needle at a 45-degree angle in one smooth motion. Release the pinch, then slowly press the plunger over 3–5 seconds.
- Withdraw and apply pressure. Pull the needle out at the same angle. Apply gentle pressure with a clean gauze or cotton ball for 10–15 seconds. Do not rub the site.
- Dispose safely. Place the used needle and syringe immediately into a sharps container. Never recap or reuse needles.
Injection site rotation: Alternate between left and right sides of the abdomen and thighs. Keep at least 1 inch between injection sites and avoid the same spot for at least 2 weeks to prevent tissue irritation.

Frequently Asked Questions
-
NAD+ is injected subcutaneously — just under the skin — using the ultra-fine 31-gauge needle included in your kit. The most common injection sites are the lower abdomen (at least 2 inches from the navel) and the upper outer thigh. You pinch the skin, insert the needle at a 45-degree angle, inject slowly, and apply light pressure afterward. The entire process takes less than 2 minutes and is virtually painless. Detailed step-by-step instructions are included with every kit.
-
The recommended protocol is 100mg subcutaneously twice per week — for example, Monday and Thursday. This maintains steady NAD+ levels rather than the spike-and-drop pattern of IV infusions. Each 1,200mg vial (6cc at 200mg/ml) provides 12 doses, or approximately 6 weeks of therapy at this dosing schedule.
-
Many patients report improved energy and mental clarity within the first 1–2 weeks. Benefits related to recovery, sleep quality, and overall vitality tend to build progressively over 4–8 weeks of consistent use. Individual results vary based on baseline NAD+ levels, age, and overall health status.
-
When prescribed by a licensed physician and sourced from a licensed compounding pharmacy, self-injectable NAD+ has an excellent safety profile. The most common side effects are mild and temporary — slight redness, bruising, or soreness at the injection site. Serious adverse events are extremely rare. Your medical history is reviewed before prescribing to ensure NAD+ is appropriate for you.
-
Oral NAD+ and its precursors (NMN, NR) are heavily degraded during digestion. Published research suggests that oral bioavailability may be as low as 5–15%. Subcutaneous injection delivers NAD+ directly into your system with near-complete absorption, providing significantly more NAD+ per dollar spent. It’s also more consistent — you know exactly how much reaches your cells.
-
Store your NAD+ vial in the refrigerator (36–46°F / 2–8°C). Keep it in its original packaging, away from direct light. Once opened, use within 28 days. Do not freeze. Before each use, inspect the solution — it should be clear and free of particles or discoloration.
-
NAD+ therapy may not be appropriate for individuals who are pregnant or breastfeeding, those with active cancer (consult your oncologist), or those with severe liver or kidney disease. All patients complete a medical intake and are individually reviewed by a physician before receiving a prescription.
-
No. Due to California State Board of Pharmacy regulations, we are unable to ship injectable NAD+ to California residents at this time. We ship to most other U.S. states.
-
Yes. Injectable NAD+ is a compounded medication that requires a prescription from a licensed physician. When you complete the online intake form, Dr. Mahjoubi reviews your health history and determines if NAD+ therapy is appropriate for you. If approved, the prescription is sent directly to the compounding pharmacy, which ships your kit to your door.
